• Proof of age:
•    o    Copy of driver’s license, ID card, or other acceptable proof of age
• Proof of income: 
•    o    Form 540, SS benefit, SSI benefit, General Relief, CalWORKS/AFDC; Notarized letter
• Proof of disability:
•    o    A recent certification (last 2 years) signed by a licensed physician
• LADWP bill, Gas bill and Telephone bill
